Docetaxel
Dosage/Direction for Use
IV infusion Breast cancer 60-100 mg/m2 once every 3 wk; 75 mg/m2 once every 3 wk (in combination w/ doxorubicin or capecitabine or when used as adjuvant therapy w/ doxorubicin & cyclophosphamide); 100 mg/m2 once every 3 wk (w/ trastuzumab). Non-small cell lung cancer 75 mg/m2 once every 3 wk. Gastric adenocarcinoma 75 mg/m2 given before cisplatin & fluorouracil; treatment is repeated every 3 wk. Squamous cell carcinomas of the head & neck cancer 75 mg/m2 given before cisplatin & fluorouracil every 3 wk for 3 cycles followed by chemoradiotherapy or 4 cycles when followed by radiotherapy alone. Hormone refractory prostate cancer 75 mg/m2 once every 3 wk w/ oral prednisone or prednisolone 5 mg bid given continuously.
Contraindications
Hypersensitivity to docetaxel & polysorbate 80. Tumor lysis syndrome.
Special Precautions
Closely monitor vital functions. Premedicate w/ dexamethasone to reduce fluid retention. Perform frequent monitoring of complete blood count. Reduce dose if grade IV neutropenia occurs. Discontinue use if severe hypotension, bronchospasm or generalize rashes occur. Peripheral neuropathy. Skin reaction eg, localized skin erythema of the extremities w/ edema followed by desquamation. Pregnancy & lactation. Childn.
Adverse Reactions
Neutropenia, anemia, febrile neutropenia, hypersensitivity, thrombocytopenia, neuropathy, dysgeusia, constipation, anorexia, nail disorders, fluid retention, asthenia, pain, nausea, diarrhea, vomiting, mucositis, alopecia, skin reactions & myalgia.
Drug Interactions
Possibility of generalized infection w/ live vaccines. Reduced clearance w/ ketoconazole & topotecan. Increased systemic exposure w/ sorafenib. Risk of increased incidence of febrile neutropenia & GI disorders (w/ doxorubicin); heart failure (w/ other cytotoxic drugs especially trastuzumab & particularly after anthracycline-containing therapy).
